MTM is a personalized service that helps you get the best results from your medications. Our pharmacist will review all your prescriptions, over-the-counter medications, and supplements to:

âś… Help you understand how and when to take your medications
âś… Discuss side effects and answer your questions
âś… Recommend cost-saving or therapeutic alternatives if needed
You may be eligible if you have Medicare Part D or Medi-Cal Rx, and especially if you:
Schedule a one-on-one session with our pharmacist
We’ll go over your full medication list
You’ll receive a written Medication Action Plan and Personal Medication Record
We stay in touch to ensure ongoing support
MTM is typically free for eligible patients with Medicare or Medi-Cal Rx coverage.
